Be the One Team Covenant:

As a member of this team, I agree to:

• Remember that I am representing Jesus Christ as well as Be the One Ministries and will model Jesus in my behavior and attitude.
• Develop and maintain a servant attitude towards all people I come in contact with during ministry and my teammates.
• Respect my Team Leader(s) and his or her decisions.
• Refrain from complaining. I know that travel can present many unexpected/undesired circumstances. Instead of complaining, I’ll be creative and supportive.
• Refrain from illegal drugs and abstain from the consumption of alcoholic beverages or the use of tobacco products.
• Maintain relationships with other team members as brothers and sisters in Christ and friendship. Not engaging in dating relationships with each other, and to keep all friendships platonic while on this trip.
• Remember that I can be sent home, at my own expense, if I do not adhere to this Covenant or if my Team Leader believes it is in my best interest or that of the team.

I do hereby release, forever discharge and agree to hold harmless Be the One Ministries Incorporated, their interns, their leaders, and their employees and officers, from any and all liability, claims or demands, which I or my heirs, executors, administrators, or assigns may have or claim to have, for personal injuries to property, real or personal, sickness, death, or unanticipated expenses of any nature which may be incurred by the undersigned participant, arising out of, or caused by, the above event or activity. I intend to be legally bound by this statement of release. Be the One Ministries reserves the right to change the dates of the Be the One Conference in the event of unpreventable schedule conflicts. If dates do change, you will be notified immediately with options for next steps. Be the One Conference Registrations are non-refundable and non-transferable. BY SIGNING... DOCUMENTATION:

1. PASSPORTS - Participants will NOT be able to leave the country without the following documents: --Ensure all of your participants have an up-to-date passport. Passports can take up to 11 weeks to come in.

2. NOTARIZED CONSENT FOR MINORS - All minors will need a consent form that is notarized by both legal guardians. Be the One has a template for this if you need one.

3. EXTRA SPENDING MONEY - Participants will be responsible for luggage fees and food before landing in mission country and after leaving mission country. BTO recommends telling the participants to bring about $150 of spending money.

4. OPTIONAL TRAVEL AND FLIGHT INSURANCE - Parents/participants have the option to purchase travel and flight insurance. Be the One is not responsible for any scheduling conflicts, flight changes, weather occurrences, physical damages, sicknesses, or losses that occur prior to or while on the mission trip.

REFUND POLICY:

1. The first payment of $250 is nonrefundable.

 

2. If you cancel after plane tickets are reserved and there is 6 weeks before departure, all can be refunded except the price of your flight and an 8% BTO administration fee.

 

4. If you cancel after 6 months before departure, none of your trip can be refunded because all of the funds will have already been allocated.

 

3. Please confirm that all details on this form are correct because if any of the information does not match a participant's passport, they will face difficulties getting through the airport. Any incorrect information will result in a ticketing name change prior to leaving the country, which usually has a fee attached to it. Be the One will NOT be responsible for fees attached to incorrect participant information.
